Performed by - AMS
Surgery time - 0 minutes

Did not have full doses of premeds remainign in safe, only able
to give Bup SR 0.4ml and Midazolam 0.16ml IM. Mask induction.
Routine prep of each carpus with betadine. Debrided each carpal
wound using a blade and tenotomy scissors to remove exuberant
granulation tissue and unhealthy wound edges, being careful to
avoid any tendons, until able to close the skin with minimal
tension. Closed each side with 5-0 PDS horizontal mattress sutures.
Post-op laser tx performed. Applied TAO and covered with tegaderm+telfa
bandages. SAK applied new foam bumpers. Recovered well, no

Removed carpal bandages. Lesions are similar in size and appearance to 3 days ago; 1cm diameter round dark pink raised tissue, with similar exudate and no odor bilaterally. Cleaned areas with dilute betadine, performed laser therapy, then applied Mupirocin ointment, Duoderm dressing, and covered with telfa + tegaderm. Left foam bumpers in place.

Feet:
Right - Removed foam noodle bandage, had some odor, likely had gotten wet. The central plantar lesion is no longer open. There is a light pink scab present with mild surrounding erythema. Cleaned the entire foot with dilute betadine, then applied a very thin layer of SSD due to odor. Placed telfa over the lesion, then covered with interdigital bandage.

Left - Removed ID bandage. Foot looks good. Replaced with new ID bandage. (this foot is wrapped to protect against development of a lesion while treating the ipsilateral pododermatitis)

Next recheck in 2-3 days

Removed carpal bumpers. Bathed carpi with Dawn to remove bandage adhesive and wound exudate from surrounding feathers, rinsed thoroughly. Less odor than previously noted with wounds. Applied lukewarm water to carpal wounds and surrounding feathers, with Dawn dish soap used to soften debris, then rinsed. Dried feathers using towel and hair dryer. Applied Mupirocin ointment and Duoderm gel patch directly to granulating wounds, then applied Tegaderm. Pool noodle bumpers applied to both wings.

Grade 1-2 pododermatitis lesion on the right foot. This appears to be worse than at presentation. Will need to monitor closely. If we cannot get this lesion to resolve and stay quiet, it is an indication that the contralateral fracture is affecting weight bearing. Placed donut bandage.

Location: Right and left carpal wounds

Performed examination and bandage change. Removed bandages from left carpus. Noted odor as bandages were removed. Wounds showed increased granulation tissue. Removed debris, scabs, cleaned gently with dilute betadine. Performed laser treatment as described above. Once dried, applied Vetericyn to wound bed, and silver sulfadiazine around periphery. Applied Tegaderm pads, foam tape, and silk tape. Repeated the process with the right carpus; right carpus had the same odor but less intense, and had one scab that was left in place due to concern for ripping the skin. Removed bandage from right foot to examine. Pododermatitis had improved, with approximately the same amount of erythema, and the central area of the pad more firm than before. Decided to leave the bandage off and reassess Monday.

Washed feet and tail. No issue around vent area. Green staining doesn't appear to be from feces, so maybe from being grounded and standing in algae covered water?

Heart rate: 250bpm
Respiratory rate: 150brpm
Auscultation: No arrhythmias on auscultation; crackles present in the right ventrolateral quadrant
Mucous membranes: Pale and moist
Dehydration: Euhydration
Comments: CRT <1s
Choana: Acute hemorrhage in choanal slit, likely secondary to confinement in kennel
Glottis: No abnormalities noted
Beak: No abnormalities noted, mild debris
Cere/nares: No abnormalities noted, no obstruction or infestation of nares
Eyes: 1+ hemorrhage OS, pinpoint hemorrhage OD
Ears: No abnormalities noted
Crop: No abnormalities noted, empty
Feather quality: Fair to good quality, mild to moderate number of tipped primary wing feathers bilaterally and across the tail
Integument: WNL, hippoboscid flies and lice present
Wings: No abnormalities noted,
Wings - Range of motion: WNL, full ROM achieved bilaterally
Wings - Patagium: Healthy and intact
Legs/Feet: Chronic, healed fracture of the left mid-shaft tibiotarsus causing varus deformitiy of the left leg; significant muscle atrophy of the left leg
Legs/Feet - Range of motion: WNL, full ROM achieved bilaterally
Legs/Feet - Plantar surface: WNL left-foot, Grade 1 pododermatitis on right plantar surface
Legs/Feet- Talons: No abnormalities noted
Neurologic: Intermittent nystagmus to the right
Ceolom: No abnormalities noted; empty
BCS: 3
Vent: No abnormalities noted; healthy mucosa
Droppings: Normal urates; green-white in color
Uropygial gland: NE
Problem list: Mid-diaphyseal chronic fracture of the left tibiotarsus with palpable callus formation, resulting in varus deformation
Hippoboscid flies and lice
Hemorrhage in the choanal slit
R-sided Nystagmus
Hemorrhage 1+ OS and pinpoint OD
Right ventrolateral crackles on respiratory auscultation
Grade 1 Pododermatitis on right plantar surface
Diagnosis: Chronic, healed L-tibiotarsal fracture
Hippoboscid flies and lice infestation
Grade 1 pododermatitis R-foot
Presumptive acute head trauma
Possible respiratory pathology - known BB seen in prior radiographs
Assessment: Overall this patient has maintained a healthy appearance and body condition despite a prior tibiotarsal fracture, this indicates he was still able to survive and obtain food in the wild and is a positive prognostic indicator. However, his ability to protect himself in the winter season is not determined
His fracture has healed abnormally and may need further assessment, imaging, and surgery if warranted.
Plan: Dx: AI testing, imaging, continue to monitor progress
Tx: Meloxicam 1mg/kg PO BID
Prognosis: Guarded
